    Skilled Nursing Care
    Physical, Occupational and Speech Therapy

  • FUNDING SOURCE(S)

    Medicare, Missouri Medicaid or Medicaid HMO (Missouri residents only, authorization required), KanCare (Kansas residents only, authorization required), Private Insurance, or VA Benefits.

  • AGE(S) SERVED

    18 and older. Under the age of 18, evaluated on a case by case basis.

  • LENGTH OF CARE

    Services take place in 60 day episodes.

  • AGENCY CRITERIA

    Service provided must be a skilled need such as nursing and/or therapy.
    Must be under the care of a physician who has ordered the treatment and services.
    Must have an illness or injury which makes it a considerable and taxing effort to leave home.

  • REQUIRES PHYSICIAN ORDER OR STATE APPROVAL?

    A physician's order is required.

Questions to ask when selecting a Home Health & Rehab Agency

WHERE CAN I FIND RATINGS ON SATISFACTION AND QUALITY CARE FOR YOUR COMPANY?

Home Health agencies are rated by the Centers for Medicare and Medicaid Services (CMS). Agencies can be rated from 1-5 stars. Medicare.gov shows Medicare Compare ratings of Home Health Agencies.

DO YOU SEE PATIENTS ON THE WEEKENDS?

Why this is important: Care should happen on your terms, not your agency’s.

What you can expect from Integrity: When you’re passionate about something, it should show.  At Integrity Home Care + Hospice, we demonstrate this commitment to your recovery to health by having a nurse on-call 24/7.  That’s peace of mind for you and your family–24 hours a day, 7 days a week.

We can also provide weekend admissions and scheduled weekend visits as ordered by your physician.

HOW SOON WILL SOMEONE BE OUT TO SEE ME ONCE I AM HOME?

Why this is important: Your road to recovery starts with the first visit.

What you can expect from Integrity: Our goal is to be in your home within 48 hours of receiving the order, however, care can begin sooner or later depending on your needs.

WHAT KIND OF TRAINING IS PROVIDED FOR YOUR CLINICIANS?

Why this is important: If your rehabilitation plan takes a detour with fluctuating/increasing care needs, your team needs to be right there beside you.  Derailing and missing the detour due to inadequate training can cause unnecessary delays in your rehabilitation and/or greater risk of re-hospitalization.

What you can expect from Integrity: We provide extensive initial and ongoing training for all staff to ensure we are consistently upholding our mission to deliver care with unmatched skill, compassion and integrity. Training includes a combination of classroom, lab, and hands-on experience to ensure that we remain competent to provide any level of care needed. We’re here with you. There for you. All along the way. (Detours included)
