Our Care Navigators will work with you to develop a holistic comprehensive plan that leaves no need unaddressed. And if we don’t offer a necessary service or support method at this time, we will find you a partner who can.

Additional Services


    From your very first call to Integrity, our Care Navigation Program helps clients and their families navigate through the complex healthcare system. Whether checking in and ensuring your plan of care is meeting your needs, working with you after hospitalization to determine the next step, or trying do decide whether hospice care is right for you and your family, our Care Navigators are with you every step of the way.

    Care Navigation is a free service available to any Integrity Home Care + Hospice client.


    Some needs are more complicated than others. Clinical Care Navigation assigns a designated RN to step in to coordinate care in the case of: multiple chronic illnesses, a terminal diagnosis, or multiple hospitalizations.


    Consumer Directed Services (CDS) is a Missouri Medicaid-based program that provides personal care attendant (PCA) services if you’re disabled and wish to live independently. You direct your own care by hiring and managing attendants to assist with daily activities.


    Hourly nursing care is designed to help you manage complex medical needs or conditions. A nurse will provide continuous care in shifts from 2 up to 24 hours a day, 7 days a week. If hourly nursing is too much, intermittent nurse visits can also be provided in addition to any other home care service, or on their own.


    If you qualify, this (IHS) is a Missouri Medicaid-based program that provides you with personal care attendant (PCA), advanced personal care attendant (APCA) and Nursing services to enable you to live an independent life.


    The importance of getting the right medication at the right time, each time, can’t be stressed enough. CarePak dispenses pre-sorted packages, clearly marked with date and time to be taken. Learn More.


    Chronic Care Management (CCM) is a care management tool used to increase quality and coordinate care between all disciplines. CCM is provided by a licensed healthcare professional on behalf of the physician/provider.


    For many people living alone, the fear of falling or becoming unable to reach for help is paralyzing. Lifeline enables individuals to live life with confidence.
