Our team of interdisciplinary specialists create a holistic care experience as you near the end of your journey, and provide ongoing support for your family.
Home Health provides you with short-term, one-on-one care after hospitalization, injury, illness or surgery. This service can complement longer-term home care services, aid in transitioning from a facility to ensure safety, improve function and prevent re-hospitalization.
CDS stands for Consumer Directed Services which is a Missouri Medicaid program that is authorized through the Department of Health and Senior Services (DHSS). This program will provide compensation to a loved one who is caring for you at home as long as that loved one is not a spouse.

Consumer Directed Services (CDS) is a Missouri Medicaid-based program that provides personal care attendant (PCA) services if you’re disabled and wish to live independently. You direct your own care by hiring and managing attendants to assist with daily activities.
Hourly nursing care is designed to help you manage complex medical needs or conditions. A nurse will provide continuous care in shifts from 2 up to 24 hours a day, 7 days a week. If hourly nursing is too much, intermittent nurse visits can also be provided in addition to any other home care service, or on their own.
If you qualify, this (IHS) is a Missouri Medicaid-based program that provides you with personal care attendant (PCA), advanced personal care attendant (APCA) and Nursing services to enable you to live an independent life.
