SPRINGFIELD, Mo. – Integrity Home Care + Hospice today affirms its appointment of Janet Weber to the position of director of strategic partnership.
As head of the Springfield business development team, Weber is entrusted with the origination of high-value partnerships to ensure seamless services across the continuum of care for individuals throughout the metro area, including Nixa, Ozark, Rogersville, Republic, Bolivar, Pleasant Hope, Buffalo, Marshfield and Strafford.
“My job every day is to build high value relationships based on credibility and trust,” she says.
Integrity Home Care + Hospice is a faith-based, privately held leader in quality, post-acute health care. Committed to quality patient and family experiences wherever people call home, the healthcare provider offers a comprehensive range of compassionate, client-centered home care, hospice, and home health rehabilitation services.
“I see Integrity as a servant-leader,” says Weber. “When you’re a servant-leader, you and your team have to walk your talk and really support the community. We have a social responsibility to ensure high quality choices are available to those who need care during serious illness or at end of life.”
As director, Weber works directly with health systems, providers and post-acute care providers, in specialties such as skilled nursing, assisted living, hospitals and rehabilitation. As needed, she and her team will directly assist in the integration of Integrity’s leading home care and hospice services with the area’s existing health care providers.
“We offer full post-acute care resources,” says Weber. “…and we have everything that can keep a patient in their home. Home is where the patient is.”
She describes a seamless continuity of care.
“The Integrity organization is Christian-based. We are highly committed to quality outcomes for patients and families,” she says, citing advantageous nurse-to-patient ratios and seamless communications among care team members.
Integrity’s commitment is reflected by high quality consumer-derived Medicare scores, Weber says. Plus, Integrity’s working partnerships directly benefit the community.
Integrity’s partners include similarly forward-thinking regional providers which meet the needs of patients who wish to remain in their home during times of serious, life altering illness or at end of life.
“We need the patient community to be educated why they and their family members need advanced directives,” she says. “We’re committed to serving as a conduit for that change.”
Partnering with Integrity allows health care providers the opportunity to address challenges in managing chronic and serious illness, customizing solutions for issues faced by the community.
“In the future, we hope to see more people accessing the hospice benefit,” she says. “Individuals who have paid into Medicare for their entire lives deserve dignity and access to a high level of quality care and pain management.”
