This form is to be used if any changes are to be made to the list of permitted individuals. Clients can call the office to confirm their current list of permitted individuals.
Description: Integrity Home Care & Hospice must obtain written authorization from the client prior to any disclosure of protected health information except as otherwise provided our HIPAA policies.
Description: Integrity Home Care & Hospice reserves the right to charge a search, retrieval, and copying fee for furnishing a client’s medical record.
Description: All clients of Integrity Home Care & Hospice has the right to access, inspect, and obtain copies of their protected health information created or maintained by the organization. However, exceptions do exist.
