Notice of Privacy Practices
Description: All clients of Integrity Home Care & Hospice are provided with an adequate notice of the organization’s legal duties with respect to protected health information.
Acknowledgement of Notice of Privacy Practices / List of permitted Individuals
Description: All clients of Integrity Home Care & Hospice are asked to provide a list of individuals involved in their care. This form is to be used if any changes are to be made to the list of permitted individuals. Clients can call the office to confirm their current list of permitted individuals.

Consent to Release Information
Description: Integrity Home Care & Hospice must obtain written authorization from the client prior to any disclosure of protected health information except as otherwise provided our HIPAA policies.

Description: Integrity Home Care & Hospice reserves the right to charge a search, retrieval, and copying fee for furnishing a client’s medical record.
Request to Access Protected Health Information
Description: All clients of Integrity Home Care & Hospice has the right to access, inspect, and obtain copies of their protected health information created or maintained by the organization. However, exceptions do exist.
