What Can I Expect?
- An Integrity healthcare professional will contact the client to explain the program one-on-one
- They will then schedule a visit to the clients home to install the equipment and teach them how to use it.
- Client will take own measurements per telemonitoring equipment- the information is automatically sent via phone line to your Integrity Home Care clinician for review.
- Clients' health status will be reviewed by an Integrity Home Care nurse daily and will initiate apppropriate clinical intervention
- An Integrity nurse liaison will report information to the client's physician
