Medical Social Worker – Hospice

Responsibilities include, but not limited to, serving as a liaison between patients and families/caregivers, patient assessments, carrying out social evaluations, and participating in development of individualized plans of care involving both the patient and family.
Candidates must be a graduate of a Bachelor’s program in social work accredited by the Council on Social Work Education; or a Bachelor degree in psychology, sociology, or other field
related to social work and is supervised by an MSW.
Must possess good verbal and written communication skills, as well as organization skills. Preferred experience in Hospice care.

Private Duty Nursing Scheduler

Our Private Duty Nursing department has an opening for a Scheduler I. Individual’s primary role will be appropriately scheduling home care visits with field staff based upon the needs of the client, employee qualifications and plan of care or treatment. This individual will also prepare and submit timely written documentation as required by policy. General Ledger Accountant

Financial Services has an opening for a full time General Ledger Accountant. Essential responsibilities include performing monthly journal entries, assisting with the creation of financial statements, and maintaining the general ledger. Human Resources Supervisor

This position assists the HR Director with planning, organizing, training and oversight of all activities of human resource management in our Lebanon office. Oversees the HR function with a strong emphasis in recruitment and hiring, personnel file management, training, compensation, employee relations, personnel investigations, and compliance with federal and state employment law, EEOC, DHSS and other state regulations.  Knowledge acquired through completion of a bachelor’s or master’s degree in human resource management, psychology, communications, business, or related field, required. On Call Registered Nurse- Hospice

The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Hours could be from 5:00pm to 8:00am. Hospice experience is preferred.

Home Health LPN

This position will provide direct patient care and related support to medically fragile clients and their families under the supervision of a Registered Professional Nurse, and within the realm of accepted standards of practice for a Licensed Practical Nurse as stated in the Nurse Practice act for the State of Missouri. Must possess a current state Licensed Practical Nurse license. Six (6) months experience as a practical/vocational nurse in a hospital, nursing home or home care setting within the last three (3) years is preferred.

Duties and responsibilities will include but are not limited to:

  • Provides assistance in the development and implementation of the plan of care.
  • Performs basic patient care and treatments including but not limited to, taking blood pressure readings, wound care, measure of vital signs, collection of blood samples, or administration of prescribed medications, in accordance with the physician’s treatment plan, individual plan of care, or as authorized by the clients payer source.

Nursing Supervisor

Integrity Home Care is seeking a Nursing Supervisor for our In Home Department. This position is in the Lebanon office.
