Home Health LPN

This position will provide direct patient care and related support to medically fragile clients and their families under the supervision of a Registered Professional Nurse, and within the realm of accepted standards of practice for a Licensed Practical Nurse as stated in the Nurse Practice act for the State of Missouri. Must possess a current state Licensed Practical Nurse license. Six (6) months experience as a practical/vocational nurse in a hospital, nursing home or home care setting within the last three (3) years is preferred.

Duties and responsibilities will include but are not limited to:

• Provides assistance in the development and implementation of the plan of care.

• Performs basic patient care and treatments including but not limited to, taking blood pressure readings, wound care, measure of vital signs, collection of blood samples, or    administration of prescribed medications, in accordance with the physician’s treatment plan, individual plan of care, or as authorized by the client’s payor’s source.

Speech Therapist

Responsible for the assessment, delivery, supervision and evaluation of speech therapy services to agency patients in accordance with the established physician's Plan of Care and in compliance with applicable federal, state and agency regulations, policies and procedures. Provides supervision to speech therapy assistants and/or home health aides who may be assigned to provide physician ordered care to the patient. Physical Therapist (PT)

Home Health has an opening for a full time pay per visit RN Case Manager for the Springfield area. Position will focus on evaluation, direction, and application of skilled nursing services. Individual must be a self-starter, possess basic computer skills, and have good organization skills. Excellent attitude and work ethic are a must! Travel required in the Springfield service area. Active Missouri RN license required. 2 years’ experience in home health setting preferred.

Pharmacy Nurse Liaison

The Pharmacy has an immediate opening for a full time Pharmacy Nurse Liaison. This individual will be responsible for developing a designated territory and will be responsible for providing efficient day-to-day coordination, delivery and marketing of pharmacy services.  This individual will also conduct client visits, pharmacy transfers, and will initiate and coordinate pharmacy education/marketing activities to internal and external resources.  This position requires the ability to triage and resolve challenges with skilled diplomacy. Human Resources Supervisor

Assists the HR Director with planning, organizing, training and oversight of all activities of human resource management in our Lebanon office. Oversees the HR function with a strong emphasis in recruitment and hiring, personnel file management, training, compensation, employee relations, personnel investigations, and compliance with federal and state employment law, EEOC, DHSS and other state regulations.

Three (3) to five (5) years’ experience managing HR department within mid-size to large organization required or equivalent/appropriate combination of HR experience and education required.

Previous experience providing functional support including HR duties required.

Professional in Human Resources (PHR) or Senior Professional in Human Resources (SPHR) certification preferred.

Social Worker

This position will provide support to IHC clients and their families.  They will maintain professional relationships with IHC and its community partner and will act as a liaison between the client and IHC as well as be a resource to meet the client’s needs.

Consumer Directed Services Specialist

The Lebanon office has an opportunity in our CDS Department for a CDS Specialist. This individual will assist persons with disabilities in accessing consumer-directed personal attendant services. This position is responsible for case management activities, information, referral services and performs record-keeping functions. Interested applicants must have a good work ethic; possess good phone etiquette, & the ability to multi-task. Candidates must be able to work independently and maintain detailed records.  Other preferred qualifications: Previous basic computer skills & a prior background in training individuals and/or marketing. This position oversees quality, cost and compliance for a team of clinicians and their respective patients.

·         Duties and responsibilities include Plans and facilitates weekly clinical Case conference. Supervisor/Team Leader

Coordinates and supervises the day-to-day care delivered by licensed and non-licensed staff in the homes of Integrity clients.  The Supervisor I acts as direct supervisor to the Scheduler I/Scheduler II, Field Supervisor(s), Team Lead(s), nurses and caregivers. 

ESSENTIAL DUTIES AND RESPONSIBLITIES: 

Maintains responsibility for the supervision, instruction, development, counseling and performance evaluation of aide/nurse staff and non-shared team members, including related documentation of such activities. Identifies, researches and resolves client and / or employee problems or concerns and reports to Director/or Manager of the division as appropriate. Schedule open visits with field staff based upon the needs of client, employee qualifications, and plan of care or treatment. Data input of schedules and client information into company tracking software as needed. Contacts “open” clients within thirty (30) minutes of any pertinent changes in their care. Responds to client/employee problems as requested by Supervisor/Manager of the Division and represents the department in a courteous and professional manner. Informs Scheduling Supervisor of scheduling changes, hotlines, performance/attendance issues, and any other problems occurring in the field. Documents schedule changes, client/employee concerns, etc. in accordance with protocol. Provides mentoring and/or counseling to employees as directed by Supervisor/Manager. Prepares and submits timely written documentation and reports as required by policy. Participates in in-service education, quality assessment activities and other staff meetings as required. Provides information to the on-call staff as necessary and directed by the Supervisor/Manager of the division. Reviews time sheets submitted by field staff for accuracy and compliance with written policy. MINIMUM JOB REQUIREMENTS
Education / Licensure:
• Graduate of an approved school of Licensed Practical Nursing.
• Licensed as a Licensed Practical Nurse in Missouri or possess Multi-State Compact License
• Ability to provide skilled and compassionate nursing care to patients and families in their home or community under the supervision of a Registered Nurse.
• Minimum of one (1) year of experience in general nursing practice preferred.
• Reliable transportation and working telephone as required by the nature of the position.

Experience: Six (6) months experience as a practical/vocational nurse in a hospital, nursing home or home care setting within the last three (3) years is preferred.

 DUTIES AND RESPONSIBILITIES

Provides personal care services and assists in activities of daily living (ADL) including, but not limited to, personal hygiene, eating, dressing and ambulation. Performs various household duties essential to health care at home including, but not limited to, disposal of infectious waste, pest control, cleaning soiled linen, simple meal preparation and housekeeping. Performs necessary errands and/or shops for essential items. Provides companionship for the client while primary caretaker is unavailable. Documents visits, assessments, and/or any significant changes in client’s condition and timely submits documentation to the appropriate personnel. Follows established departmental policies, procedures, and objectives. Practices safety, environmental, and/or infection control measures. May provide Advanced Personal Care (APC) services, if APC certified, to include, but not limited to assistance with: ostomy care, catheter care, bowel program, medications, range of motion and using client transfer devices, such as a Hoyer lift. Performs miscellaneous job-related duties as assigned by Supervisor.
