Director of Clinical Services - Hospice

Our Hospice Department has an opening for a full time Director of Clinical Services. This individual will be responsible for ensuring that patient care is coordinated and managed appropriately, receive case referrals, review and evaluate each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians to promote more effective performance and delivery of quality home care services.  They will also assist with screening and interviewing process of new employees and make recommendations for employment of individuals. Assess the health needs of community and assist in the development of plan(s)/ program(s) to meet needs, also participates in public relation and community activities that promote the organization's role as an effective member of the hospice and health care delivery system.  Previous experience in hospice/home care setting with two (2) years management or supervisory experience.  

RN Case Manager

Home Health has an opening for a full time pay per visit RN Case Manager for the Springfield area. Position will focus on evaluation, direction, and application of skilled nursing services. Individual must be a self-starter, possess basic computer skills, and have good organization skills. Excellent attitude and work ethic are a must! Travel required in the Springfield service area. Active Missouri RN license required. 2 years’ experience in home health setting preferred.

Pharmacy Nurse Liaison

The Pharmacy has an immediate opening for a full time Pharmacy Nurse Liaison. This individual will be responsible for developing a designated territory and will be responsible for providing efficient day-to-day coordination, delivery and marketing of pharmacy services.  This individual will also conduct client visits, pharmacy transfers, and will initiate and coordinate pharmacy education/marketing activities to internal and external resources.  This position requires the ability to triage and resolve challenges with skilled diplomacy. Hospice CNA

The hospice aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort.  The hospice aide is responsible for observing the patient, reporting these observations and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care.

Hospice RN

The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience.  The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Consumer Directed Services Specialist

The Lebanon office has an opportunity in our CDS Department for a CDS Specialist. This individual will assist persons with disabilities in accessing consumer-directed personal attendant services. This position is responsible for case management activities, information, referral services and performs record-keeping functions. Greater Kansas City Area

Human Resources Manager

Assists the HR Director with planning, organizing, training and oversight of all activities of human resource management throughout our Kansas City offices.  Oversees the HR function including recruitment, hiring, personnel file management, training, compensation, employee relations, personnel investigations, and compliance with federal and state employment law, EEOC, DHSS and other state regulations.

Knowledge/Education:

Knowledge acquired through completion of a bachelor's or master’s degree in human resource management, psychology, communications, business, or related field, required. Understanding of employment law including, but not limited to, EEOC regulations, FLSA, ADA, ADEA, PDA, OSHA, MHRA, and FMLA required. Proficiency with MS Word, Excel, Outlook and internet applications required.

Experience/Certification: 

Three (3) to five (5) years’ experience managing HR department within mid-size to large organization required or equivalent/appropriate combination of HR experience and education required. Previous experience providing functional support including HR duties required. Professional in Human Resources (PHR) or Senior Professional in Human Resources (SPHR) certification preferred.

Supervisor/Team Leader

Coordinates and supervises the day-to-day care delivered by licensed and non-licensed staff in the homes of Integrity clients.  The Supervisor I acts as direct supervisor to the Scheduler I/Scheduler II, Field Supervisor(s), Team Lead(s), nurses and caregivers. 

ESSENTIAL DUTIES AND RESPONSIBLITIES: 

Maintains responsibility for the supervision, instruction, development, counseling and performance evaluation of aide/nurse staff and non-shared team members, including related documentation of such activities. Identifies, researches and resolves client and / or employee problems or concerns and reports to Director/or Manager of the division as appropriate. Schedule open visits with field staff based upon the needs of client, employee qualifications, and plan of care or treatment. Data input of schedules and client information into company tracking software as needed. Contacts “open” clients within thirty (30) minutes of any pertinent changes in their care. Responds to client/employee problems as requested by Supervisor/Manager of the Division and represents the department in a courteous and professional manner. Informs Scheduling Supervisor of scheduling changes, hotlines, performance/attendance issues, and any other problems occurring in the field. Documents schedule changes, client/employee concerns, etc. in accordance with protocol. Provides mentoring and/or counseling to employees as directed by Supervisor/Manager. Prepares and submits timely written documentation and reports as required by policy. Participates in in-service education, quality assessment activities and other staff meetings as required. Provides information to the on-call staff as necessary and directed by the Supervisor/Manager of the division. Reviews time sheets submitted by field staff for accuracy and compliance with written policy. Prepares reports on service delivery, mileage expenditures, employee requirements (evaluations, OJT, In-Service training) for review by the Departmental Manager/Director. Regional Client Advocate

This position will provide a variety of advocacy services to better support our current lines of business, by closing the gap on communication and services, decreasing onboarding time, and providing resources and education to potential clients.  

ESSENTIAL DUTIES AND RESPONSIBILITIES

·         Assist in obtaining doctor orders for Home Health & In-Home, as needed, to resolve communication issues and build relationships.

·         Manage in-process referrals from Client Services to ensure they turn into admissions at the divisional level on a timely basis.

·         Assist clients in processing paperwork for Veterans to expedite the process and reduce the amount of calls.

·         Serve as a resource connection in assisting individuals with navigating the qualification process of Medicaid or community based services

·         Identify and develop referral targets with heavy emphasis on relationship and strategic marketing techniques of Medicaid related services.

·         Consistently visit identified referral sources and identify new opportunities

·         Provide Medicaid documentation processing and continued follow-up to potential clients in their home.

·         Serve as a resource/liaison for operational departments of Integrity Home Care who may experience challenges with various referral sources and provide intervention and                            communication as requested

·         Work as liaison for clients with potential referrals that need physical follow-up to reduce the amount of outstanding time onboarding.

·         Follow up with non-compliance and provide continual navigation to support these clients to move them through the process with success if at all possible.

·         Cross refer services from staff on behalf of clients and make home visits as necessary to assist in providing a partnership in care and identify resources that may be of assistance             in increasing the quality of life and care in the home.

·         Provide face to face grievance follow up in partnership with the regional manager and department manager.

·         Provide education to the community with clear guidelines and expectations of locations and contacts with focus on Medicaid services and Continuity of Care.

·         Assist Social Worker with local resource calls such as; food pantry, transportation, home repair, utility assistance, and documentation follow up.

·         Attend weekly Home Health Case conferences and Home Care Consultant Team conference calls to cross refer service lines for clients as needed

·         Serve as a backup for Consumer Directed Services, In-Home, and Private Care openings.

·         Serve as backup for Quality Assurance calls for the Private Care Division and Client Annual Visits.

·         Serve as a back-up for hospital visits. 

·         Conducts client surveys utilizing the information gained to assist in addressing client-specific as well as systemic issues. Scheduler

We have an opportunity in our Business Development Department for a full-time Home Care Consultant. The primary responsibilities of these positions are community education and referral generation. Individuals who have a strong background in sales are encouraged to apply. Applicants must be dynamic, self-motivated and be willing to make business to business phone calls on a daily basis. Home Health RN

MINIMUM JOB REQUIREMENTS

Home Health LPN

This position will provide direct patient care and related support to medically fragile clients and their families under the supervision of a Registered Professional Nurse, and within the realm of accepted standards of practice for a Licensed Practical Nurse as stated in the Nurse Practice act for the State of Missouri.

Duties and responsibilities will include but are not limited to:

  • Provides assistance in the development and implementation of the plan of care.
  • Performs basic patient care and treatments including but not limited to, taking blood pressure readings, wound care, measure of vital signs, collection of blood samples, or    administration of prescribed medications, in accordance with the physician’s treatment plan, individual plan of care, or as authorized by the client’s payor’s source.
  • Observes, documents and timely reports any changes in the client’s condition to the appropriate personnel and/or physician.
  • Follows established departmental policies, procedures, and objectives.
  • Practices safety, environmental, and/or infection control measures.
  • May supervise In-Home Aides for Advanced Personal Care (APC) services.
  • Performs miscellaneous job-related duties as assigned by Supervisor.

MINIMUM JOB REQUIREMENTS

Reliable transportation and working telephone as required by the nature of the position.

Education/Licensure:  Graduation from an approved School of Practical/Vocational Nursing and current state Licensed Practical Nurse license.

Experience:  Six (6) months experience as a practical/vocational nurse in a hospital, nursing home or home care setting within the last three (3) years is preferred.

Physical Therapist

Provides direct patient care and related support to medically fragile clients and their families under the supervision of a Registered Professional Nurse, and within the realm of accepted standards of practice for a Licensed Practical Nurse as stated in the Nurse Practice Act for the State of Missouri.

DUTIES AND RESPONSIBILITIES
• Provides assistance in the development and implementation of the plan of care.
• Performs basic patient care and treatments including but not limited to, taking blood pressure readings, wound care, measure of vital signs, collection of blood samples, or administration of prescribed medications, in accordance with the physician’s treatment plan, individual plan of care, or as authorized by the client’s payor’s source.
• Observes, documents and timely reports any changes in the client’s condition to the appropriate personnel and/or physician.
• Provides ongoing instruction and education of client’s health care plan to family and or other health team personnel.
• Follows established departmental policies, procedures and objectives, including Infection Control Policies and Compliance Plan.
• Practices safety, environmental, and/or infection control measures.
• May supervise Personal Care Aides for Advanced Personal Care (APC) services.
• Ensures communication of information to appropriate team members.
• Submits timely documentation per agency policy.
• Participates in in-service programs and staff meetings.
• Evaluates own needs for support and using identified system(s) to meet the need.
• Complies with HIPAA Policies. Maintains confidentiality of information relating to the patient and family. Will discuss only those aspects necessary to the care and treatment of patient and family with those directly involved in the patient’s care.
• Complies with all state and local regulations, the Nurse Practice Act.

MINIMUM JOB REQUIREMENTS
Education / Licensure:
• Graduate of an approved school of Licensed Practical Nursing.
• Licensed as a Licensed Practical Nurse in Missouri or possess Multi-State Compact License
• Ability to provide skilled and compassionate nursing care to patients and families in their home or community under the supervision of a Registered Nurse.
• Minimum of one (1) year of experience in general nursing practice preferred.
• Reliable transportation and working telephone as required by the nature of the position.

Experience: Six (6) months experience as a practical/vocational nurse in a hospital, nursing home or home care setting within the last three (3) years is preferred.
